As a key member of our Sutter Care at Home team, you will play a vital role in managing new referrals, facilitating timely admissions, and providing exceptional customer service to patients, families, and referral sources.

Key Responsibilities:
- Manage new referrals and ongoing follow-ups, ensuring timely and consistent admissions
- Communicate effectively with patients, families, and referral sources, maintaining clear and concise communication
- Collaborate with referring hospitals, physicians, board and care facilities, skilled nursing facilities, and other healthcare providers
- Enter referral information into the Electronic Medical Records (EMR) system and create initial orders to facilitate the start of care
- Utilize and promote excellent customer service skills in all interactions
- Collaborate with referral center staff, home health, Advanced Illness Management (AIM), and hospice staff across offices
Requirements:
- Graduate of an accredited school of Vocational Nursing
- Unencumbered LVN-License Vocational Nurse license for the state you reside
- Must have or obtain an LVN-License Vocational Nurse of California within 3 months of hire (company-sponsored and assisted)
- 1 year of recent relevant experience
