We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Case Manager. In this role, you will utilize your clinical expertise to evaluate the medical needs of our members, develop proactive care plans, and collaborate with interdisciplinary teams to address complex health and social indicators. Your exceptional analytical, problem-solving, and communication skills will enable you to drive positive outcomes and enhance the overall wellness of our members.
Key Responsibilities:
- Conduct comprehensive assessments to identify member needs and develop personalized care plans
- Utilize clinical tools and data analysis to inform care decisions and drive member engagement
- Collaborate with interdisciplinary teams to address complex health and social indicators
- Apply motivational interviewing skills to empower members to take an active role in their healthcare
- Utilize case management processes in compliance with regulatory and company policies and procedures
Requirements:
- Active and unrestricted RN licensure in the state of Florida or compact licensure in state of residence
- 3+ years of clinical experience
- 2+ years of Case Management, Discharge Planning, and/or Home Healthcare Coordination experience
- Bachelor's degree or minimum of an Associate's degree with equivalent experience
Preferred Qualifications:
- Certified Case Manager
- Excellent analytical and problem-solving skills
- Bilingual desired (English / Spanish or English / Creole)
- Effective communications, organizational, and interpersonal skills
- Ability to work independently
- Effective computer skills including navigating multiple systems and keyboarding
