Job Overview
As a Registered Nurse (RN) - Home Health Care Specialist with Jefferson Health at Home by BAYADA, you will have the opportunity to use your clinical skills and compassionate heart to treat patients where they want to be the most - in the comfort of their own home. You will be responsible for delivering 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plans, including administration of medication, wound care, treatments, and comfort care. Your role will also involve performing client assessments, providing education to patients and/or their families, and monitoring patients' conditions.
Key Responsibilities
- Learn and expand your clinical skills caring for clinically complex level patients in the home.
- Perform 5-6 visits per day in a small geographical territory, focusing on 1:1 care with each patient.
- Deliver high-quality, patient-centered care that meets the physical, emotional, and social needs of our patients.
- Collaborate with our interdisciplinary team to develop and implement individualized care plans that promote patient independence and dignity.
- Provide education and support to patients and their families, empowering them to take an active role in their care.
- Monitor patients' conditions, identify potential health risks, and develop strategies to mitigate these risks.
- Maintain accurate and timely records of patient care, including assessments, interventions, and outcomes.
Essential Qualifications
To be successful in this role, you will need:
- An active and current license to practice as a professional registered nurse (RN) in the state of PA.
- At least one year of clinical experience, with previous home healthcare experience welcomed but not required.
- The ability to deliver patient care with compassion, excellence, and reliability, as you would want those you love to be treated.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Basic life support certification.
Preferred Qualifications
While not essential, the following qualifications are highly desirable:
- Previous experience in home health care or a related field.
- Electronic medical records (EMR) experience.
