Job Summary

We're looking for a highly skilled and compassionate Certified Medical Assistant to join our team as a Chronic Care Management Specialist. As a key member of our telehealth team, you'll be responsible for providing exceptional patient care, coaching, and education to individuals managing chronic conditions. Responsibilities

• Triage patient calls and provide timely responses to their inquiries and concerns.
• Conduct regular condition assessments to identify areas for improvement and develop personalized care plans.
• Collaborate with patients to set and achieve health goals, providing education and coaching on disease management and self-care.
• Perform monthly medication reconciliations to ensure accurate and up-to-date medication lists.
• Develop and implement patient-centered care plans that address their unique needs and goals.
• Communicate effectively with healthcare providers to ensure seamless care coordination and transitions.
• Provide education and support to patients on disease management, self-care, and healthy lifestyle habits.

Qualifications

To be successful in this role, you'll need to possess the following qualifications and skills:

• CMA or RMA certification (required)
• Prior experience in Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) (preferred)
• Bilingual Spanish (preferred)
• Strong knowledge of third-party payer requirements and electronic medical records
• Excellent organizational, problem-solving, communication, and interpersonal skills
• Ability to work independently and as part of a remote team
• Strong attention to detail and ability to maintain accurate records

Preferred Qualifications

We're looking for candidates with the following preferred qualifications:

• 1+ year of experience in RPM or CCM
• 1+ year of experience in telehealth
• Certified Medical Assistant Certificate (CMA or RMA)
• Bilingual Spanish

Compensation and Benefits

We offer a competitive hourly rate of $18.00 - $21.00 per hour, depending on experience. As a 1099 contractor, you'll have the flexibility to work as many or as few hours as you'd like, with a minimum of 30 hours per week. We also offer a flexible schedule, with day shifts available.
