Key Responsibilities:
- Provide comprehensive social and medical case management support to members across the U.S.
- Manage and prioritize a caseload of member cases, ensuring timely and effective interventions.
- Conduct thorough assessments to determine member eligibility for case management services.
- Connect members with appropriate resources, including specialty programs, educational materials, and community support services.
- Conduct research and provide guidance on member issues, including referrals to legal, financial, and community support services.
- Collaborate with colleagues and clinical staff to achieve optimal member outcomes.
- Maintain accurate and complete documentation of all member interactions and case details.
- Participate in quality improvement initiatives to enhance member care.
Qualifications:
- Proven experience in social and medical care.
- Strong customer service and communication skills.
- Proficiency in using computers and navigating various systems.
- Bachelor's degree or equivalent experience.
Preferred Qualifications: Prior experience in managed care, psychology, or social work; Bilingual abilities.
