Job Summary and Key Responsibilities As a Medical Social Worker, you'll be responsible for: * Assessing patients' social and emotional states, identifying their needs, and developing personalized plans of care to support their recovery and well-being * Evaluating patients' relationships, medical and nursing needs, financial resources, and available community resources to inform care decisions * Providing education and support to patients and families on coping with social and emotional responses to illness or injury * Collaborating with the interdisciplinary care team to develop and implement comprehensive plans of care * Participating in discharge planning, care coordination, and community resource utilization * Supervising and evaluating the performance of Social Work Assistants (BSW) as required Requirements and Qualifications To be successful in this role, you'll need: Education: Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education Licensure: Social Worker licensure in the state of practice (if required by state law or regulation) Experience: Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice Skills: Excellent oral and written communication, interpersonal skills, and ability to work effectively with patients, families, and healthcare teams Certifications: Proof of current CPR certification, valid driver's license, auto insurance, and reliable transportation Preferred Qualifications and Skills We're looking for candidates with: Knowledge: of discharge planning needs, community resources, and eligibility for state/federal financial and medication programs Ability: to organize tasks, develop action plans, set priorities, and function under stressful situations Flexibility: to work varied shifts, including evenings and weekends, and travel locally as needed Language: fluent English language skills What We Offer: Competitive Salary, Comprehensive Benefits, and Growth Opportunities As a valued member of our team, you'll enjoy: Competitive Salary: $52,500 - $72,225 per year, depending on experience and qualifications Comprehensive Benefits: medical, dental, and vision coverage, 401(k) retirement savings plan, time off, and disability insurance Growth Opportunities: professional development, training, and education to support your career goals Flexible Scheduling: varied shifts, including part-time and per-diem opportunities Why Join Our Team?
