JOB RESPONSIBILITIES: • Principally responsible for the coordination of activities related to the care coordination and management of patients needing perinatal care, preventive health and chronic disease management. • Provides triage for women's health/prenatal patients, conferring with clinical providers as needed. • Responsible for reviewing the current patient data prior to each office visit for laboratory, x-ray and other test results. • Must maintains accurate and timely documentation of all pertinent data in medical record. • Prepares patient for examination and sets-up any necessary equipment. • Evaluates patient's care needs through assessment, observation and comparison to established WHN protocols and standards. • Responsible for organizing patient care needs and ensuring continuity of care in conjunction with the clinical provider. • Administers in-office ordered medications while documenting appropriately. • Maintains patients' immunization logs by documenting pertinent data. • Obtains blood samples and performs all CLIA certified or waived tests as ordered by medical provider. • Schedules all patient referrals for specialty and diagnostic services and testing procedures and provides documentation with prior authorization information. • Under the direction of the medical provider, answers telephone requests by patients by documenting appropriate patient information. • Assists with the ongoing maintenance of patients' medical records. • Provides chronic disease and preventive screening education to patients, families, partners, and internal staff or external agencies. • Works with appropriate WHN staff to assist patients in accessing support services to facilitate financial assistance and entitlement applications processes. • Coordinates outreach efforts to communicate with patients care for preventive health needs and /or chronic disease management. EDUCATION, EXPERIENCE, AND TRAINING: Educational requirements are an Associate's degree or higher in Nursing. Minimum of(4) years of experience in an ambulatory care or family practice clinic, a community health center, a public health setting or a health care related environment. Knowledge of prenatal and women's health procedures and care coordination concepts. Knowledge of eligibility requirements and available services from the BCCP and the LRD.
