As a Social Worker at Your Health, you will be responsible for:
- Initiating Advance Care Planning for patients and completing comprehensive assessments and care plans
- Coordinating discharges, placements, and continuation of coverage for patients
- Building and maintaining professional relationships with referral sources, patients, and families
- Documenting patient interactions and maintaining accurate records
- Meeting daily productivity goals and demonstrating excellent time management and organization skills
To succeed in this role, you will need:
- Excellent customer service skills and ability to communicate effectively with patients, families, and healthcare teams
- Strong computer technology skills, including EMR experience and ability to navigate multiple software applications
- Time management, organization, and placement skills
- Ability to work independently and as part of a team in a remote setting
