As part of our growing division, we are seeking an experienced Advanced Practice Provider to join our team of compassionate and innovative healthcare professionals. By traveling to the homes of our patients, our Geriatric providers are able to develop strong relationships with their patients and families, keeping them healthy and thriving in the comfort of their own homes.
As an In-Home Geriatric Nurse Practitioner, you will have the opportunity to work autonomously while maintaining ownership of your patient panel. Your daily responsibilities will include providing in-home primary care to our most frail and at-risk elderly patients, caring for an average of six to eight patients daily, and providing coverage for Metrowest communities. You will be an integral part of a cohesive team, working together to deliver high-quality patient care and making a significant difference in the lives of our patients.
To be successful in this role, you will need to possess a strong foundation in geriatric care, as well as excellent communication and interpersonal skills. You will be a graduate of an Accredited Nurse Practitioner Program, with a Master's level degree preferred. You will also be licensed by the Commonwealth of Massachusetts board of registration in Nursing and maintain BLS certification, professional certification as appropriate. Additionally, you will be fluent in EMR systems and technology, with a strong ability to navigate complex healthcare systems.
