Schedule: Full-time, 40 hours per week Primary Job Responsibilities: • Validates and submits registry harvest reports to meet regulatory and registry compliance guidelines. • Ensures data accurately depicts performance for publicly reported measures to CMS and other accrediting bodies. • Maintains accurate databases rigorously to ensure follow-up deadlines are met according to registry requirements. • Coordinates institute quality and outcomes committees or clinical projects with hospital and physician leadership to improve quality and value. • Completes longitudinal patient follow-up by interviewing and assessing patient health status across the continuum to meet regulatory and registry requirements. • Navigates complex patient needs during assessments and refers to appropriate providers for further care. • Identifies and enrolls clinical registry candidates according to national registry guidelines.
