- Yorke and Northern Local Health Network, Port Pirie - Located in the Northern Aboriginal Health team - Salary - Multi-classed  - RN/M2C - $90,913 to $114,917 p.a. + Superannuation + leave loading + salary sacrifice benefits - AHP2 - $91,785 to $106,330 p.a.* + Superannuation + leave loading + salary sacrifice benefits - Full Time Contract - Ongoing - Eligibility Open to Public Sector Employees Only * Regional Incentives may apply, subject to the terms in the Enterprise Agreement Lead Chronic Disease Prevention in the Aboriginal community across the Northern YNLHN region Drive the prevention, early detection and management of chronic conditions in the Aboriginal community through culturally safe, high-quality care.  The Chronic Disease Coordinator plans, implements and evaluates programs and services, working with multidisciplinary teams and stakeholders to improve health outcomes.  The role also provides clinical guidance to support staff and ensures services are responsive to community needs.
