Lead Chronic Disease Prevention in the Aboriginal community across the Northern YNLHN region
Drive the prevention, early detection and management of chronic conditions in the Aboriginal community through culturally safe, high-quality care. The Chronic Disease Coordinator plans, implements and evaluates programs and services, working with multidisciplinary teams and stakeholders to improve health outcomes. The role also provides clinical guidance to support staff and ensures services are responsive to community needs.
Key Responsibilities:
- Deliver and manage culturally safe, holistic primary health care
- Plan, implement and evaluate care models and chronic disease programs
- Coordinate projects, programs and research to improve outcomes
- Collaborate with multidisciplinary teams and external stakeholders
- Advocate for Aboriginal and Torres Strait Islander health needs
- Provide clinical guidance and support to students and less experience staff
Skills & Attributes:
- Strong communication, problem-solving, negotiation and conflict resolution skills
- Ability to work effectively in a multidisciplinary team
- Ability to prioritise workload, meet deadlines, and work with minimal supervision
- Creativity, innovation and flexibility in approaching healthcare challenges
Essential Qualifications:
Nursing/Midwifery stream (RN/M2)
- Registration with the Nursing and Midwifery Board of Australia with current practising certificate
- Minimum 3 years post-registration experience or current Clinical Nurse/Midwife classification
- Competence in relevant nursing practice and experience supervising students or less experienced nurses
Allied Health stream (AHP2)
- Recognised qualification in relevant Allied Health profession
- Eligible for membership/registration with professional association; commitment to CPD
