Role Description This role involves providing patient care support for a centralized nursing program, outpatient and home care services, including Triage services, Anti-coagulation, and refill. Individuals in this role will work in an outpatient clinic setting. • Collects, prioritizes and synthesizes comprehensive data pertinent to the patient's health or situation. • Analyzes assessment data to determine nursing diagnoses. • Identifies expected outcomes individualized to the patient. • Develops a plan that prescribes interventions to attain expected outcomes. • Implements the identified plan in a safe, timely, appropriate manner. • Evaluates the patient’s progress towards attainment of the outcome. • Participates in quality improvement activities related to nursing practice. • Attains knowledge and competency that reflects current nursing practice. • Evaluates one’s own nursing practice in relation to professional practice standards and regulatory guidelines. • Collaborates with patient, family, and others in the conduct of nursing practice. • Acts in an ethical manner and maintains a therapeutic and professional patient-nurse relationship. • Integrates research findings in practice. • Provides leadership in the professional practice setting and the profession. • Practices in an environmentally safe and healthy manner. • Demonstrates ability to coordinate and direct unit operation when acting as Charge Nurse. • Demonstrates ability to identify and plan appropriate learning experiences when acting as Preceptor.
